The Medi-Cal system covers DME items when medically necessary and prescribed by a physician. Each product has differing coverage criteria. The basics are a related diagnosis and medical necessity.

Products that are under $100.00 are billed directly without the need for authorization.

Most products require an authorization from the Medi-Cal field office. This is called a Treatment Authorization Request (TAR). In the TAR process the physicians order, and medical records (tests, chart notes, etc) are forwarded to Medi-Cal for approval. The process usually takes under 30 days.
