| Follow up |
You’ll find that patients referred to Diamond are far less likely to experience an emergency room visit related to their equipment. We’re able to maintain our stellar record by ongoing, carefully planned, follow up. Beginning at the time of delivery the patient’s environment is evaluated. A care plan is developed and it is handed off to our follow up team. For respiratory patients, they are visited by a respiratory therapist within the first 48 hours of receiving their equipment. They receive a full evaluation, titration (when ordered), and additional training. A report is forwarded to the treating physician. All other patients receive a call from our follow up team within 72 hours of initial setup. A carefully planned series of questions helps us determine if any additional training may be required. If so, it is dispatched for same day servicing. Additional follow up calls occur at varying intervals depending on the equipment delivered. Preventative maintenance is also scheduled according to the products requirements. In the event a problem is discovered that is unrelated to our equipment, the patients care team is notified immediately. |
