Eocene for Home Health Agencies

Eocene is a telemedicine system. It consists of a glucometer and a cradle. The cradle connects to the home telephone line, just like an answering machine. Each night the patient places their glucometer into the cradle and it transmits the test results to our HIPPA compliant server.


Once the test results are received, they are scanned for potential problems (hyper / hypo). You tell us your acceptable range. If the results are outside of that range you are notified immediately. The system also reports to you when a patient is not testing or is encountering errors.


These alerts allow the home health agency, the physician, and Diamond to intervene before a minor problem becomes a hospital admission.


The Eocene acts as your eyes in the patients home, 24/7. It allows nurses to focus their time on more critical patients and still monitor their diabetic census.  This technology is instrumental in maintaining (and raising) O.A.S.I.S scores.
